CHH Care Managers and Social Workers provide the information, resources, and assistance needed to complete recovery beyond a patient's hospital stay. Care Managers are experienced Registered Nurses skilled in assessment, treatment, and care planning, and are located in all inpatient and outpatient areas and units at the Hospital and at Hungerford Emergency and Medical Care in Winsted.

When you are admitted to Charlotte, you are assigned a Care Manager or Social Worker who begins to assess care, evaluate your daily progress, and monitor plans to ensure you are on schedule to be discharged from the Hospital on the expected release date.
