The CHH Social Services Department plays a key, and increasingly important role in managing the patient’s care throughout their hospitalization and after care.  Its Mission is to facilitate the achievement of optimal quality, and clinical outcomes, patient access to care and appropriate utilization of resources, balanced with the patient’s right to self-determination. Our Charlotte Hungerford Social Workers will address the health, spiritual, cultural, emotional, financial and discharge needs of our patients, families, and community while promoting positive clinical, cost effective and quality outcomes.

The CHH Social Services Department is comprised of RN’s, Social Workers and Case Management Assistants.  Care Managers are experienced RN’s skilled in assessment, Treatment and Care Planning. These Nurses coordinate the care of patient’s throughout their hospitalization.  They provide education, information, resources and assistance to complete the patient’s recovery during and beyond the hospital Stay. Care Managers are located in all inpatient and outpatient areas and units at the Hospital and at Hungerford Emergency and Medical Care in Winsted, Connecticut.

Care Manager’s work closely with a patient’s Doctor and Health Care Team to initiate the appropriate discharge plan. This plan may be Home Health Care, Acute Rehabilitation, Skilled Nursing Facility, Long Term Care or Hospice Care. They arrange Meals on Wheels, order Durable Medical Equipment, and arrange transportation and Intravenous Therapy if indicated. They follow-up with their patient’s after discharge confirming the plan is working well and address any additional needs that have come up.

Social Workers assist our patient’s with financial concerns and social needs. They help patient’s access funds for prescription coverage. They provide counseling and support to patients and caregivers coping with a new diagnosis, chronic illness or end of life issues.  Our Social Workers provide after care referrals to various community resources, including mental health providers, substance abuse treatment programs, housing and shelters. They also  assist new mothers and babies access the care and social services they need and provide information on living wills and advance directives to all who request.
