Common Issues

“Enrollment Error”

There are several reasons you received an “Enrollment Error”

Your Medical Record Number could be incorrect

The most common mistake is with the case sensitivity of the H at the beginning of the number.  The H is case sensitive and must be capitalized when entering.  Check that and retry.

Your E-Mail Address “does not match” or “isn’t on file”  with the medical records office.

To successfully enroll, you must use/match the e-mail address that is on file with the Medical Records Office here at CHH.  If you suspect that it is not on file, please call (860) 496-6670 for instructions on how to correct this.

Your E-Mail Address conflicts with one that’s already on file with CHH

The system does not accommodate duplicate e-mail addresses – each patient in the system must have a unique e-mail addressIf you share an e-mail address with another portal user, you will need to use a different email address OR have a hospital staff member enroll you.

 

Different Browsers

Chrome, Firefox, Safari & Internet Explorer

The system will work with the current version of most browsers, i.e. Chrome, Firefox, Safari & Internet Explorer.  NOTE – If you are using Internet Explorer 6 or older, you must update to IE version 7 or later.
